Completed a session yesterday. Opened the app today and there are two approval cards from that session waiting. Clicking Approve does nothing. Clicking Reject does nothing. They reappear on refresh.

That's an orphaned approval - the parent session completed but the approval record didn't get cleaned up. Usually happens if the Lambda got interrupted right after executing the action but before writing the 'approval resolved' state.
Short-term fix: go to the session itself (Sessions > find the one referenced in the card), click the ... menu > Clear Orphaned Approvals.
Long-term: the scheduled cleanup Lambda that sweeps orphaned approvals hourly was added to the infra a few days ago. Shouldn't see new ones going forward. Drop us a note if you do.
